Hazing Reporting Form

The university accepts anonymous reports. However, the university seeks to provide supportive measures for all parties involved which is not possible with an anonymous report. A reporter should not fear a loss of privacy by making a report because it allows the university to discuss the options available to them as well as supportive measures, which are available regardless of the level of involvement in the report.
